"* Derived type enhancements: parameterized derived types, improved control of accessibility, improved structure constructors, and finalizers.

* Object-oriented programming support: type extension and inheritance, polymorphism, dynamic type allocation, and type-bound procedures.

* Data manipulation enhancements: allocatable components (incorporating TR 15581), deferred type parameters, VOLATILE attribute, explicit type specification in array constructors and allocate statements, pointer enhancements, extended initialization expressions, and enhanced intrinsic procedures.

* Input/output enhancements: asynchronous transfer, stream access, user specified transfer operations for derived types, user specified control of rounding during format conversions, named constants for preconnected units, the FLUSH statement, regularization of keywords, and access to error messages.

* Procedure pointers.

* Support for IEEE floating-point arithmetic and floating point exception handling (incorporating TR 15580).

* Interoperability with the C programming language.

* Support for international usage: access to ISO 10646 4-byte characters and choice of decimal or comma in numeric formatted input/output.

* Enhanced integration with the host operating system: access to command line arguments, environment variables, and processor error messages."

and there is 2008 version in the wings.

I agree that innovation and economies of scale do not account for the cost differential between the US and other countries. I think there are a number of factors that contribute to the higher cost of US health care. I also noted, "Both of the above books describe barriers to change and a failure of competition to drive change." In the near term, healthcare reforms may reduce total costs by removing barriers to change and improving competition. In the long term, innovation and economies of scale will provide continuous improvement to make health care more efficient and avoid cost increases.

Does that mean that other countries have lower health care costs because they have more innovation and larger economies of scale? I would be surprised if that were the case.

Even if they drop the maximum speed down to that of Fios b is out there would far outdo anything else that is out there. That would bring it comparable to HSDMA overseas. The problem lies with which of the carriers wants to pony up and extend the limit of data throughput but with speeds that fast the throughput limit is going to become forefront to the debate. Even at 1 cent per megabyte, running at 20 Gb will cost you a lot of pennies per minute. I don’t think that it will be so much an idea of if but when and who will do it first. . I am guessing AT&T due to their later launch date and the need to come out swinging to play catch up to Verizon.

This takes a lot of work on the back end and is the general reason that they did not want the FCC to OK a lot of the better technology in the past. But this is here, it’s public and it is going to be big. Something has got to give…
